Filing Commendations
If you wish to commend a member of DVA Ambulance, write a letter, or fill out our online commendation form. You can also download our commendation form (below) and mail it to us.

Your remarks will be shared with the employee, and his or her supervisors. A copy will be placed in the employee’s permanent employment record, and may be considered during performance evaluations.
