
Filing a Concern DVA Ambulance, Inc. investigates allegations of employee misconduct and alleged violations of policy, procedure, and law.
If a formal investigation is opened, Management will assign a supervisor and the supervisor will contact you. They will give you his or her name and phone number. You can call this person Monday through Friday between 9 am - 5 pm, if you have questions or concerns. What Happens After I File a Concern? The Supervisor will ask you to give a written statement.
